หมายเหตุ

1. นักศึกษาที่ไม่ได้ลงทะเบียนให้ยื่นคำร้องฯ ภายใน 60 วัน นับจากวันเปิดภาคการศึกษา มิฉะนั้นจะถูกให้พ้นสภาพนักศึกษา 
2. นักศึกษาที่ลงทะเบียนและประสงค์ลาพักในภาคการศึกษานั้น ๆ ให้ยื่นคำร้องที่ได้รับอนุมัติที่สำนักทะเบียนฯ ก่อน 1 สัปดาห์ของวันแรก ของวันแรกของการสอบไล่ประจำภาค
3. นักศึกษาที่ลงทะเบียนเรียบร้อยและได้รับอนุมัติให้ลาพักการเรียน จะได้รับอักษา W ทุกรายวิชา

Remark

1. Student Who did not enroll this semester must submit the form within 60 days, after the first day of school. Otherwise, the status will be cancelled.
2.  Student who enrolled for courses have to submit the approved copy to the Registrar's office on week prior to the first day of final exam weeks.
3. Student who enrolled for courses and whose petition is approved will receive W in all courses.

คำร้องนี้ใช้สำหรับนักศึกษาที่คาดว่าจะสำเร็จการศึกษา และไม่มีรายวิชาที่จะต้องลงทะเบียนเรียน
